You can locate a facility in the area of your choice by entering a zip code, city, or county which will then detail the location with a map of the surrounding area.
The website identifies the Skilled Nursing Facility; Intermediate Care Facility; Intermediate Care Facility - Developmentally Disabled, Intermediate Care Facility - Habilitative; Intermediate Care Facility - Developmentally Disabled Nursing, Congregate Living Health Facility, Pediatric Day Health/Respite Care Facility, General Acute Care hospitals, Acute Psychiatric hospitals licensed by the California Department of Public Health.
The website provides profile information for each facility such as ownership, certification status (acceptance of Medicare and/or Medi-Cal) and performance history including complaints; entity reported incidents, state enforcement actions, and survey deficiencies.
